• Photocopy of NRIC
• Certified True Copy SPM/ O-Level/ Equivalent
• Passport Sized Photograph
• Medical Statement (If any)
• Registration Fees

CHECKLIST FOR PTPTN

BEFORE STUDENT APPLY ONLINE

• PTPTN PIN number from Bank SimpananNasional (RM5)
• Bank Islam Savings Account
• SSPN Account (min RM 20)
• Student email address
• Parent particulars ( full name, IC number, income per month – gross & net pay after deduction must be the same as in the salary slip)
• SPM / STPM / UEC / A Level / Foundation (CGPA) / Diploma result (CGPA)
• Course Info – Diploma
• Intake date – must be same with the offer letter
• Matriks No : Student ID No. eg DEM1816456

AFTER GETTING ONLINE PRE-APPROVAL

• 2 duty stamps – RM10 each
• Complete the agreements 2 sets (sign and write date on the required fields-use black/blue ball pen for both).
• Date must be same for both agreements.
• 1 set of PTPTN Online Application Summary (Log in and click the "CetakMaklumat" link to print).
• Offer Letter – which will be prepared by Registry
